MEDICAL INSURANCE:  1P920
CLAIMS PROCEDURES FOR ROUTINE AND

NON-EMERGENCY MEDICAL EXPENSES:
The employee should complete an SSQ Inpatriate Claim Form and Consent Form.
The employee should attach all original receipts for expenses being claimed.
If a claim for any sickness or injury exceeds $500.00, the Attending Physician’s Statement on the claim form must be completed by the physician.
The employee submits the forms and receipts to the appropriate plan administrator or directly to SSQ.
SSQ processes the claim and makes the claim payment payable to the Insured and sends it to the appropriate plan administrator or directly to the Insured.

FOR SIGNIFICANT EXPENSES INCURRED DUE TO A MEDICAL EMERGENCY/NECESSITY:
The employee or care provider should contact the Accident & Sickness Claims Department in Calgary, Alberta at 1-855-772-3082 and ask for either Alicia McGillvary.   Please provide the policy number and details of the medical emergency.

An Inpatriate Claim Form and Consent Form should be completed by the Insured and submitted to the plan administrator or directly to SSQ as soon as possible.  Late claim submission from the Insured would hamper our ability to provide prompt payment to those care providers who have sent invoices directly to us.  If any expenses have been paid by the Insured, the receipts should be attached to the claim form.  Please ensure they indicate the nature of the illness or injury.

SSQ will provide reimbursement of all eligible expenses to the care providers.  If the employee has paid any or all care providers and appropriate receipts have been submitted, we will provide reimbursement to the Employee.

ACCIDENTAL DENTAL AND EMERGENCY DENTAL: 1P920 CLAIMS PROCEDURES:

For Emergency dental (the employee is in discomfort and requires treatment) the Standard Dental Claim Form should be completed.

For Accidental dental (a blow to the mouth), the Accidental Dental Claim Form should be completed.
